Form 1 - Claim Form

HOW TO MAKE A CLAIM

In order to make a claim the parent must pay all medical expenses incurred as a result of treatment
and get receipts.

The parent must then compete the claim form-I, attach a claim form but a claim form can always be downloaded
from the website-www.jfd.ie.

If the claim is over €500 the parent must obtain a Medical Certificate completed by the attending
doctor/dentist.

It is important that the parent fill in their own Bank Details in section 7 on the claim form as AIG will settle the
claim directly into their Bank Account.

The parent must then email the completed Claim Form, receipts and Medical Certificate-if applicable- to
irelandclaims@aig.com.

The claim will be settled into the designated Bank Account within 10 working days provided everything is in
order.
